Part 3: Equipment Acceptance Agreement

If your equipment grant application with PIFBS is approved, you will be responsible for the following. Please review and check each box to continue.

The equipment received through this grant is the property of the program listed above. Should there be staffing changes, the equipment is expected to stay with the school program. PIFBS will not be able to provide replacement equipment or fulfill additional grant requests within the same calendar year.*

I will provide PIFBS with photos of my players and the donated items. Photos are required through the season, and some are required to be sent by April 15, 2024.*

I will secure appropriate permissions for use and sharing of photos. PIFBS uses these photographs and letters to promote its charitable efforts with donors and prospective partners in a variety of mediums (i.e. social media, website, annual report). Photos received will be published on social media platforms of PIFBS.*

I will complete an Impact Report. Impact reports are vital to PIFBS securing additional funding/equipment from our partners. At minimum, your report should include at least 1 individual player impact story with a matching photo. Reports should be completed at the end of your season, early reports will not be accepted.
